PRIVATE LESSON PARTICIPANT FORM

You may bring up to five guests to your private lesson.  Please enter information below for each guest at least 24 hours ahead of your scheduled lesson time.

Participation will require each guest to have a pass or lift ticket that is valid on the same day as the lesson.
